The DOT exam is not meant to fail anyone: it is meant to confirm you can drive safely. Even so, many drivers leave with a short certificate or have to come back for a document. This guide, written for truck drivers, explains how to arrive prepared, what shortens the certificate and what to do afterward with DPS.
How do I prepare to pass the first time?
- Sleep well the night before and do not arrive rushed. Stress and lack of sleep raise blood pressure, and blood pressure is the number one cause of short certificates.
- Skip coffee, energy drinks and cigarettes in the two hours before, for the same reason.
- Take your medications as usual. Stopping them "so it does not show on the exam" is what makes the exam go badly.
- Bring your glasses or hearing aid. The requirement is 20/40 vision in each eye and hearing a forced whisper at 5 feet, with correction allowed.
- Use the restroom right before only if you already gave your urine sample. The test looks for protein, blood or sugar; it is not a drug test.
What shortens or denies the certificate?
The certificate lasts up to 24 months, but the medical examiner can issue it for less when something needs monitoring. The most frequent reasons:
- High blood pressure. From 140/90 it usually means a one-year certificate; from 160/100, three months so you can get it under control; at 180/110 you are not certified until it comes down.
- Diabetes. With pills and a good A1C you are certified; with insulin you need form MCSA-5870 completed by your treating physician within the last 45 days.
- Sleep apnea. If you use a CPAP, bring the usage report; untreated, the examiner may require a sleep study before certifying.
- Uncorrected vision problems, hearing loss without an aid, or a recent heart condition without cardiologist clearance.
What documents do I bring?
A valid license, a medication list with doses, and depending on your case: a recent A1C result, the insulin form, the CPAP report, a letter from your cardiologist or specialist, and your previous certificate. If you have an FMCSA exemption (vision, diabetes or limbs), bring it too.
What do I do with the certificate afterward?
In Texas, DPS requires you to file your medical certificate to keep your CDL active: it is submitted at a driver license office or through the DPS online portal along with your self-certification of driving type. If the certificate expires without renewal, the CDL can be downgraded to a regular class. Note the expiration date and schedule the renewal a month ahead.
Can I come back if I did not pass?
Yes. If blood pressure was high, the reading can be repeated the same day or at a short visit after a few days of control. If a document was missing, the exam is completed when you bring it. The examiner tells you exactly what you need to be certified, and treatment for high blood pressure or diabetes can start at the same clinic.
