Which pregnancy test is done, and when is it reliable?

The urine test detects the pregnancy hormone (hCG) and is reliable from the first day of a missed period; done earlier, it can be negative even if there is a pregnancy. The blood test detects the hormone a few days before the urine test and, in its quantitative version, measures the amount, useful for dating or monitoring an early pregnancy. The physician tells you which one fits your dates.

What happens if the test is positive?

The result is confirmed, the due date is estimated from your last period and your history is reviewed: blood pressure, diabetes, medications you take and previous pregnancies. Folic acid and prenatal vitamins are started that day, initial lab work is ordered if you wish, and an ultrasound is scheduled between 6 and 8 weeks to confirm the pregnancy is in the uterus and see the heartbeat. What if it is negative but the period does not come?

The test is repeated in a week or the blood test is done. If it stays negative, other causes of the delay are looked for: stress, weight changes, thyroid, polycystic ovaries or perimenopause. A thyroid and hormone test usually clears it up, and follow-up is done under gynecology.

When should I come right away?

If with a positive test you have bleeding, strong pain on one side of the belly, dizziness or fainting, come the same day or go to the emergency room: a pregnancy outside the uterus must be ruled out. Vomiting that keeps you from holding down fluids also needs care.
