Which chronic conditions are managed at the clinic?

Type 2 diabetes and prediabetes, high blood pressure, high cholesterol and triglycerides, and the ones that usually come with them: excess weight, fatty liver and thyroid problems. We see both the new diagnosis and the patient who already takes medication and needs follow-up, dose adjustment or a second opinion on their treatment.

What is the first follow-up visit like?

Your history and medications are reviewed, blood pressure, weight and glucose are taken, and the baseline tests are ordered: A1C, lipid panel, kidney and liver function, and urinalysis. If the physician sees fit, an EKG is done at the same visit. With results in hand, your targets and plan are set: diet, activity, medications and the date of the next check.

Which targets are used?

For most adults: A1C under 7%, blood pressure under 130/80, LDL cholesterol under 100 and triglycerides under 150. Targets are adjusted for age and other conditions; the physician tells you yours and writes them down to compare at each visit.

How often should I come?

While treatment is being adjusted, every 4 to 8 weeks. Once the numbers are on target, every 3 months with an A1C if you have diabetes, and every 3 to 6 months for blood pressure and cholesterol. Once a year the full labs, foot and eye checks and seasonal vaccines are repeated. No appointment is needed for any of these visits.

Which medications are managed?

First-line ones and their adjustments: metformin and other oral diabetes drugs, insulin when needed, blood pressure medications and statins. If a medication does not agree with you or misses the target, it is changed at the visit. Always bring your bottles to check doses and avoid duplicates.

How much does follow-up cost in Pasadena?

A follow-up is charged as an office visit; labs depend on the panel.
