How do I know if I have a urinary infection?

The typical symptoms are burning or pain when urinating, needing to go often with little output, urgency, cloudy or foul-smelling urine, and pressure in the lower belly. It is far more common in women because of anatomy. If there is also fever, chills, pain in the lower back or one side, or nausea, the infection may have reached the kidney and needs to be seen that same day.

Which test is done?

A general urine test with a dipstick and in-clinic analysis, which detects white blood cells, blood and nitrites, the signs of infection. When the infection recurs, there is fever, you are a man, you are pregnant or you have diabetes, a urine culture is also sent, which identifies the bacteria and which antibiotic it responds to. The sample is taken midstream, after cleaning the area, so it is not contaminated.

What treatment is given?

An antibiotic chosen according to your symptoms, your history and, if there is one, a previous culture; for uncomplicated cystitis it usually lasts 3 to 7 days. It is important to finish it even if the burning is gone by the second day. Plenty of fluids and something for pain are also advised if needed. If you do not improve in 48 to 72 hours, or get worse, come back: the bacteria may be resistant and the antibiotic may need to change based on the culture result.

Why does my infection keep coming back?

The most common causes are holding urine, drinking little water, sex without urinating afterward, poorly controlled diabetes, hormonal changes of menopause and, in men, prostate enlargement. Three or more infections a year deserve a workup: culture, glucose and, in some cases, an ultrasound of the urinary tract.
