Physical exams for school and sports, fast and with the forms completed.

- Complete physical exam
- Vital-signs check
- School and sports forms completed
- Care in Spanish
Every August brings the same rush: the school or team form, the physician's signature and a deadline running out. At Clínica Hispana Nueva Salud Pasadena the school and sports physical is done with no appointment, in Spanish or English, and you leave with the form completed and signed the same day.
Which forms are completed?
The pre-participation physical evaluation used by Texas schools (the UIL form for school sports), the entry physical some schools and daycares require, and forms for camps, leagues and programs. Bring the form printed or on your phone; if your school did not give you one, the physician issues a physical exam certificate.
What does the physician check?
Health history and family history, with special attention to fainting, chest pain with exercise, asthma, allergies, previous injuries and early cardiac deaths in the family; vital signs with weight, height and blood pressure; vision and hearing; heart and lungs; abdomen; spine and joints; and a general growth check. If something needs workup, such as a murmur, the physician orders it before signing.
Is the sports physical the same as the school physical?
Almost. The sports physical focuses on the heart, breathing and injuries, because its goal is for the student to train safely; the school physical is a general growth and health check. Both can be covered at the same visit and both forms completed.
How often is it needed?
The school sports physical in Texas is renewed every year, usually before the season. The entry physical is required when starting a new school or stage. It is best done weeks in advance: if something needs workup, there is time to resolve it without losing enrollment.
What should I bring?
The school or team form, with the parent or guardian section already filled out and signed; the adult's ID; the child's vaccine record, if the school asks for it; glasses or hearing aid if used; and the medication list, including the inhaler. A parent or guardian accompanies minors.
How much does a school physical cost in Pasadena?
We give you the price by phone at (281) 747-8817 before you come. No health insurance is needed; we accept cash and cards. If the student also needs a lab test or a vaccine, it is done at the same visit and quoted separately.
